Mounjaro Dosage & Titration Schedule

To minimise side effects, you must strictly follow the titration ladder. You cannot jump straight to a high maintenance dose, even if lower doses are harder to find elsewhere. You must always wait for the clinically correct strength.

The 4-Week Rule: Use each dose strength for at least 4 weeks before moving up. If you tolerate a dose well and are losing weight, you can choose to stay on that dose rather than increasing.
Titration Path: How to Increase Doses
Treatment PhaseDosagePurpose
Month 1 (Weeks 1-4)2.5 mgInitiation: Gets your body used to the medicine.
Month 2 (Weeks 5-8)5 mgTherapeutic: The first dose effective for weight loss.
Month 3 (Weeks 9-12)7.5 mgStep-Up: Increases efficacy if needed.
Month 4 (Weeks 13-16)10 mgMaintenance: A common long-term dose.
Month 5+12.5 mg / 15 mgMaximum: Only if lower doses stop working.

What to expect: weight loss week by week on Mounjaro

Weeks 1–4 (2.5 mg dose)
During your first month, your body is simply adjusting to the new medication. While you might experience some mild side effects as your system adapts, you should also start to notice your appetite and daily cravings beginning to reduce. Significant weight loss is unlikely at this introductory stage, so it is best to focus on establishing a healthy routine and staying hydrated.

Weeks 5–8 (5 mg dose)
As you step up to the first therapeutic dose, the physical changes become much more pronounced. You will likely experience far fewer cravings and find yourself feeling fuller for much longer after meals. This makes portion control feel significantly easier to manage, and many patients report their daily energy levels improving as they settle into better eating habits.

Months 3–6 (Doses increasing up to 15 mg)
Moving through the titration schedule, you can expect much better overall appetite control and a noticeable reduction in distracting "food noise". This is the period where steady, visible weight loss typically occurs. By the end of your sixth month, it is quite common to have safely lost between 9% and 11% of your starting body weight.

Six months onwards (Maintenance dose)
Once you reach your ideal long-term maintenance dose, your newly adjusted eating habits will feel naturally controlled, leaving you with better, more consistent energy. When supported by healthy lifestyle changes, patients can lose an incredible average of 22.5% of their starting weight after 18 months, as demonstrated in the SURMOUNT-1 clinical trial.

Please remember that every body responds differently to medication, meaning individual weight loss results and timelines will naturally vary.

Switching to Mounjaro from Wegovy or Ozempic

Many patients ask us about switching treatments. Because Mounjaro uses a dual-action formula (GIP + GLP-1), it works differently from Wegovy (GLP-1 only).

  • No Direct Swap: You cannot switch directly from 2.4mg Wegovy to 15mg Mounjaro.
  • Washout Period: We recommend a 5-week break between your last injection of Wegovy/Ozempic and your first Mounjaro injection.
  • Reset to Start: You must start Mounjaro at the 2.5mg dose to let your body adjust to the new GIP hormone.

What is Mounjaro?

Mounjaro is a once-weekly self-injection designed to help manage type 2 diabetes and support weight loss in adults living with obesity or a high BMI. The active ingredient in this medication is tirzepatide, which works seamlessly with your body's natural systems to help regulate your blood sugar and significantly reduce your appetite over time.

Unlike older weight management medications, Mounjaro is a dual GIP and GLP-1 receptor agonist, meaning it mimics two distinct metabolic hormones simultaneously. By activating both the GLP-1 and GIP pathways, the medication powerfully slows your digestion and reduces the hunger signals sent to your brain. This innovative dual action is exactly what differentiates Mounjaro from single-hormone treatments like Wegovy and Saxenda, offering a uniquely comprehensive approach to your weight loss journey.

Who manufactures Mounjaro?

Mounjaro is manufactured by Eli Lilly, a globally established pharmaceutical company founded in 1876 with a long-standing reputation for advancing metabolic medicine. Eli Lilly initially launched Mounjaro in 2022 as a highly effective treatment for type 2 diabetes before its substantial weight management benefits were fully realised in clinical trials.

After comprehensive research demonstrated its safety and effectiveness for obesity, the medication became officially MHRA-approved in the UK in November 2023. Because it is strictly regulated by the Medicines and Healthcare products Regulatory Agency (MHRA), you can feel completely confident that the authentic medication you receive meets the highest medical standards required within the United Kingdom.

Mechanism of Action

  • Suppresses Appetite: Signals the brain to reduce hunger and "food noise".
  • Slows Digestion: Keeps you feeling full for longer after meals.
  • Balances Blood Sugar: Improving insulin sensitivity helps prevent sugar spikes that trigger cravings.

Studies like Nauck & D'Alessio (2022)[2] suggest this dual action makes Mounjaro more effective than single-hormone treatments like Wegovy or Ozempic.

How long does Mounjaro take to work?

Mounjaro starts working from your very first dose, though you will likely feel the internal changes before you see significant drops on the weighing scale. During your first week of taking tirzepatide, it is quite common to notice a rapid decrease in your hunger and fewer cravings for snacks. By the end of your first month, this appetite suppression usually becomes much more noticeable as the dual GLP-1 and GIP receptor agonists begin to steadily regulate your blood sugar and slow your digestion. However, do not be discouraged if your initial weight loss is relatively slow; the starting dose is primarily designed to safely introduce the medication to your body rather than trigger immediate fat loss.

The full, therapeutic effects of the treatment will gradually build as you step up your dose over the coming months. Because every metabolism is completely unique, individual responses will naturally vary from person to person. To achieve the best possible results, you simply need to stay consistent with your weekly injections and allow the medication sufficient time to work alongside your healthier lifestyle habits.

Clinical Results

In the SURMOUNT-1 clinical trial[1], participants on the highest dose lost an average of 22.5% of their body weight over 72 weeks. Most users notice appetite suppression within the first 4 weeks.

Research also shows long-term benefits. High doses of Mounjaro achieved a 25.3 percent reduction within 88 weeks in maintenance studies[3], and real-world data confirms superior weight loss compared to semaglutide[4].

The most common side effects reported were mostly mild to moderate gastrointestinal issues, such as nausea and diarrhoea, which typically subsided as the body adjusted to the medication. Ultimately, these rigorous studies demonstrated both Mounjaro's exceptional short-term efficacy for weight reduction and its crucial long-term benefits for maintaining that weight loss over time.

Who is Eligible? (BMI Requirements)

Eligibility Criteria To Buy Mounjaro In The UK including BMI chart

To buy Mounjaro in the UK, you must meet specific Body Mass Index (BMI) criteria:

  • BMI 30+: Suitable for obesity management.
  • BMI 27+: Suitable if you also have a weight-related condition (e.g., prediabetes, high blood pressure, high cholesterol).

Unsure if you qualify? Read our Beginner's Guide to Mounjaro.

Important Safety Warnings

Mounjaro is not suitable for everyone. Do not use this medicine if:

  • You have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia type 2 (MEN 2).
  • You are pregnant, breastfeeding, or planning to conceive (stop Mounjaro at least 1 month before trying for a baby).
  • You have a history of pancreatitis.
  • You have severe digestive problems, such as gastroparesis.
  • You have Type 1 diabetes.

Diabetic Retinopathy: Tell your prescriber if you have eye problems, as rapid blood sugar changes can temporarily affect vision.

Who can't take Mounjaro?

Mounjaro is not suitable for everyone, and certain individuals cannot take this medication due to specific medical histories or age restrictions. The treatment is strictly prescribed for adults between the ages of 18 and 75, as those over 75 may experience an increased risk of adverse side effects. Furthermore, you must not use this injection if you are allergic to tirzepatide or any of its underlying ingredients, or if you are currently pregnant or breastfeeding.

To ensure your complete clinical safety, our prescribing team will not issue Mounjaro if you have:

  • A personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • Type 1 diabetes.
  • A history of pancreatitis or severe digestive conditions, such as severe gastroparesis.

Additionally, if you have a history of diabetic retinopathy, you will need to discuss this carefully with your prescriber before beginning treatment to monitor your vision. Finally, if you are currently taking other diabetes medications, such as insulin or sulphonylureas, please consult a healthcare professional first; combining these treatments can significantly increase your risk of experiencing low blood sugar (hypoglycaemia).

Can I get Mounjaro on the NHS?

Yes, Mounjaro (tirzepatide) is approved for use on the NHS for weight management, but access is currently limited to patients meeting strict National Institute for Health and Care Excellence (NICE) criteria during a multi-year phased rollout.

While NICE guidance (TA1026) recommends the medication, simply having a high Body Mass Index (BMI) does not automatically qualify you for an NHS prescription. Access is prioritised for those with the highest clinical need who are already engaging with specialist NHS weight management services.

What are the NHS criteria for Mounjaro?

As of 2026, to be eligible for Mounjaro on the NHS, you must typically meet a combination of BMI thresholds and specific health complications. You generally need to be referred to a Tier 3 Weight Management Service or a multidisciplinary team (MDT) and meet the following conditions:

  • BMI Threshold: A BMI of at least 35 kg/m² (or 32.5 kg/m² for members of Black African, African-Caribbean, Asian, Chinese, or Middle Eastern ethnic groups).
  • Comorbidities: You must have at least one weight-related comorbidity, such as hypertension (high blood pressure), dyslipidaemia (high cholesterol), obstructive sleep apnoea, or cardiovascular disease.
  • Prior Treatment: Evidence that conventional diet and exercise interventions have been unsuccessful.
Please Note: The NHS rollout is phased over several years (starting with the highest BMI cohorts). Even if you meet the criteria above, local Integrated Care Boards (ICBs) may have waiting lists or caps on prescriptions due to high demand.

Mounjaro Private vs NHS: What is the difference?

Many patients choose private prescriptions through services like UK Meds to bypass long waiting lists or because they do not meet the strict NHS "Phase 1" or "Phase 2" rollout criteria.

Recommended Diet and Lifestyle Changes on Mounjaro

To achieve the best possible results on Mounjaro, you must combine your weekly injections with a balanced, reduced calorie diet and a regular exercise program. While the medication helps to significantly reduce your appetite and curb cravings, your long-term success relies on building sustainable, healthy habits that nourish your body as you lose weight.

Because you will be eating smaller portions naturally, it is vital to follow a calorie controlled diet[1] that prioritises high-quality, nutrient-dense foods. To help prevent muscle loss and keep your energy levels steady, here is a quick guide on how to build a supportive meal plan:

Exercise and Ongoing Support

Increasing your daily physical activity is just as important as adjusting your eating habits. Finding a routine you genuinely enjoy, whether that is brisk walking, swimming, cycling, or weight training, will boost your metabolism and improve your overall cardiovascular wellbeing.[7]

To keep yourself accountable, consider using digital trackers and plans to monitor your meals, hydration, and daily movement. If you are unsure of where to start or how to train safely, it is always a good idea to seek professional dietary and physical activity advice tailored to your specific fitness level and medical history.

Remember, weight loss injections are most effective when utilised as part of a dedicated weight management service or a comprehensive weight loss package.[6] By engaging with these wraparound support systems and focusing on your broader lifestyle, you give yourself the very best tools to achieve and maintain a healthier weight for life. Remember to consult a medical professional before making any significant changes to your lifestyle.

Side Effects and Safety

Monalisa Kapinga - Pharmacist and Clinical Safety Officer - GPhC Number 2232744

Independent pharmacist and Clinical Safety Officer Monalisa Kapinga explains the most common side effects and how to manage them:

"Mounjaro may cause digestive, metabolic and general side effects, especially during the early weeks of treatment. Nausea, vomiting, diarrhoea or constipation are among the most common changes. Eating smaller meals, avoiding rich or spicy foods and staying hydrated can help. Burping or stomach discomfort may improve by eating slowly and avoiding fizzy drinks.

Seek urgent medical help if you experience signs of gallbladder issues, severe abdominal pain or vomiting, which may indicate pancreatitis. A rash, swelling of the face, lips or tongue, or breathing difficulty may suggest an allergic reaction. Stop Mounjaro and seek emergency care if this happens."

Like all medicines, Mounjaro can cause side effects, although not everybody will experience them. Below is a breakdown of the known side effects categorised by how frequently they typically occur.

Very common (may affect more than 1 in 10 people):

  • Nausea
  • Diarrhoea
  • Constipation
  • Vomiting
  • Stomach pain
  • Reduced appetite

Common (may affect up to 1 in 10 people):

  • Fatigue and dizziness
  • Indigestion, heartburn, and bloating
  • Burping and flatulence
  • Injection site reactions (such as redness or mild swelling)
  • Gallstones
  • Hair loss
  • Mild allergic reactions (such as a skin rash or itching)
  • Low blood pressure

Uncommon (may affect up to 1 in 100 people):

  • Injection site pain
  • An altered sense of taste
  • An increased resting heart rate
  • Acute gallbladder disease
  • Pancreatitis: Characterised by severe stomach pain radiating through to your back. Seek emergency medical help immediately if you suspect this.

Rare (may affect up to 1 in 1,000 people):

  • Anaphylaxis: A severe allergic reaction. If you experience difficulty breathing or rapid swelling of your face, lips, or tongue, please call 999 immediately.

How long do side effects last?

Most Mounjaro side effects last for just a few days to a couple of weeks as your body gets used to the medication. You might find that mild gastrointestinal issues, such as nausea or bloating, temporarily return each time you step up your dose on the titration schedule. However, these symptoms should quickly settle down again as your system adapts. If your side effects remain persistent, become unmanageable, or cause you significant distress, you should always speak to your prescriber for tailored medical advice. You can also read more on our blog: How long do Mounjaro side effects last?

How to reduce side effects

You can reduce your Mounjaro side effects by staying well-hydrated and eating smaller, bland meals as your digestive system adjusts to the treatment. It is absolutely vital that you strictly follow your prescribed dosing schedule and never rush your dose increases, as skipping steps often triggers unnecessary illness. If you do experience discomfort, over-the-counter remedies like basic antacids or mild laxatives can provide effective short-term relief. For full safety details, refer to the official Patient Information Leaflet (PIL).

Alternative Weight Loss Treatments

While Mounjaro is highly effective, we also prescribe other licensed treatments. Wegovy (semaglutide) and Saxenda (liraglutide) are popular alternatives. Ozempic is also available for type-2 diabetes management.

Is it legal to buy Mounjaro online in the UK?

Yes, provided you use a registered service like UK Meds. Our prescribers review every order. If approved, we issue a private prescription (included in the cost) and dispense the medication from our partner GPhC-registered pharmacy.

Can I drink alcohol on Mounjaro?

Alcohol does not directly interact with tirzepatide, but it can lower blood sugar and worsen nausea. It is best to limit intake, especially when you first start treatment. Read more: Can you drink alcohol on Mounjaro?

Does Mounjaro affect the contraceptive pill?

Because Mounjaro slows stomach emptying, it might affect how oral contraceptives are absorbed. It is recommended to use an additional barrier method of contraception (like condoms) for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.
